The lead retrieval form holds the questions a booth team answers after they scan an attendee's badge, such as a lead score and notes. In Eventify you set one global form for the event, and you can give any exhibitor or sponsor its own version when they need different questions to qualify leads.
When a booth team member scans a badge, the lead is saved and the team can answer the lead form to record how interested the person is. The answers are stored with the lead and appear when the exhibitor or sponsor reviews and exports their leads.
There are two levels:
Editing the global lead form depends on your Eventify plan. If your plan doesn't include it, the Update Global Lead Form menu item shows an upgrade icon and doesn't open the form. Click the icon to see the upgrade options. See How to Upgrade or Renew Your Eventify Plan.

The lead score question can't be deleted. Every exhibitor and sponsor that uses the global form sees your changes. Companies that have their own custom form keep their version.

The Lead Form tab now shows "Custom form active". To change the custom form later, open the same tab and click Update Lead Form under Edit Form.
From the same tab you can still change the global form. Click Update Global Lead Form under Edit Global Form. This changes the form for every company that uses it.
The Lead Form tab only appears when you edit an existing exhibitor or sponsor, not while you add a new one.

You'll see "This exhibitor's lead retrieval form has been reset successfully!" The company then uses the global form again.
Only reset when the company no longer needs its custom questions. As the warning says, resetting the custom form results in loss of the data that belongs to it.

Do exhibitors and sponsors share the same lead retrieval form?
Yes, they share one global lead form. You can give any single exhibitor or sponsor a custom form with Customise For This Exhibitor or Customise For This Sponsor.
Will editing the global lead form change custom forms?
No. Companies with a custom form keep it. Only companies that use the global form see the change.
Why can't I open Update Global Lead Form?
If the menu item shows an upgrade icon, your plan doesn't include lead form configuration. Upgrade your plan to edit it.
Why is there no Lead Form tab when I add an exhibitor?
The tab only shows when you edit an exhibitor or sponsor that already exists. Save the company first, then open it again.
