The lead retrieval form is the set of questions your team answers when qualifying a lead, such as the lead score, notes and anything else you want to record. You can build your own version of the form in the portal.
Open the Lead Form
- In the left sidebar, open Leads.
- Click Lead Form.
The page shows a Form Preview of your current form. If you haven't customised it yet, this is the event's default form.

Open the form builder
Click Edit Form. The builder opens full screen, with the fields you can add on the left, your form in the middle, and the field settings on the right.
The top bar has:
- The × to close the builder, and the form title. Click the title to rename the form.
- Desktop, tablet and mobile buttons to check how the form looks on each.
- Preview to try the form, and Exit Preview to go back.
- Save.

Add a field
- In the Add Field sidebar, pick a field type. Under Input Fields you have Text Input, Number Input, Multi-line Input and Voice Note. Under Selection Fields you have Checkbox, Radio Button, Dropdown and Date.
- Drag it onto the form, or click it to add it.
- Drag fields up or down on the form to change their order.
Set up a field
- Click a field on the form to select it.
- In Update Field Settings, enter the Label (the question your team sees) and the Variable (a short name for the field). Both are required.
- Tick Mark it mandatory if your team must answer it before saving a lead.
- Tick Exportable in scanned lead data to include the answer in your lead export. New fields have this ticked already.
- For Checkbox, Radio Button and Dropdown fields, fill in the Options and Values rows.
- For Date fields, choose Single Picker or Range Picker, and a Date Restriction: Any date, Before a date, After a date or Between two dates.
- Click Save in the panel. You'll see "Field updated".
To delete a field, select it and click its trash icon. The lead score field can't be deleted, and its Variable can't be changed.
Save the form
Click Save in the top bar. It shows "Saving..." and then "Form saved successfully!".
The first time you save, the portal keeps your version as your company's own form. The event's default form stays as it was for everyone else. From then on, your team sees your form when qualifying leads in the Lead details panel.
Messages you might see
- "This field is required.": Fill in the Label or Variable.
- "Variable name is already used by another field.": Give the field a different Variable.
- "Special characters removed. Only letters, numbers, underscore and spaces are allowed.": The Variable can only contain letters, numbers, underscores and spaces.
- "End date must be after start date.": Pick an end date later than the start date.
- "Required." or "Duplicate." on an option: Fill in the empty option, or change one that repeats another.
- "Failed to save form": Your form wasn't saved. Click Save again.
