If you choose the ‘Survey Form’ option as ‘Mandatory’ and enable the button, then the user will get a survey form displayed while navigating the app and he/she has to fill up the form to proceed further.
However, many disable this button and prefer not to include the survey form for their users.

Choose the Survey Form, and click on Edit or you can create a new one.
Go to Form Builder, where you can easily edit your form, just drag and drop your chosen options, reorder them and insert them in your form.
Go to Form Settings, and enable the Resubmit Form button and your users will be able to submit their feedback multiple times while accessing the app. But if this is disabled, the survey form can be submitted only once.
